CHW & Community Program Evaluation

Community Health Workers and community-based teams do far more than what is visible in a checkbox or a billing code. Ensightics designs evaluations that reflect the full scope of their work including relationship-building, advocacy, and systems navigation—alongside traditional outcome measures.

This service area is dedicated to CHW initiatives, Community Violence Intervention (CVI) efforts, and other community-based programs where trust, access, and connection are central to impact.

How Ensightics can help

  • Clarify roles, pathways, and expected outcomes for CHW and community-serving programs
  • Measure changes in access, navigation, trust, engagement, and wellbeing, not just clinical indicators
  • Integrate data from EHRs, program databases, and community data collection tools, when feasible
  • Design participant and CHW surveys, as well as interview and focus group guides
  • Document relationship-building, advocacy, and systems navigation in ways that can be shared with funders and partners
  • Create dashboards and summary tools for supervisors, funders, and CHWs themselves
  • Prepare evaluation sections and letters of support for CHW and CVI related grant proposals
  • Support learning conversations that center CHW and community perspectives in interpreting results
